De term "Grim Reaper" betekent in het Engels "Magere Hein", en de Carolina Reaper is dan ook bijna dodelijk heet. Zelfs liefhebbers van extreme hitte zouden geen hele peper in één keer moeten eten (en volgens de kweker alleen onder begeleiding). Mensen die niet gewend zijn aan scherpe smaken, moeten er zeker niet aan beginnen.
Ze zijn nauwelijks geschikt om puur te eten, maar wel perfect voor het maken van chilisaus, drogen, inmaken of invriezen.
